
Monitor My Health is a minority-led nonprofit providing evidence-based health coaching and prevention programs, to underserved CT residents. The proposed project will adapt and expand MMH’s existing office building into a 21,406-square-foot mixed-use development, combining expanded administrative and program space with 19 affordable senior housing units designed to support wellness and aging in place. The resident housing will be combined with on-site health education, chronic disease screenings, and access to registered dietitians, to directly address healthcare accessibility and housing stability.
